You have to contact Blizzard. What happens is if you don't log in for a long time, the authenticator ends up being out-of-sync with the account and Blizzard has to reset it.

The gal I talked to said either to call or email them.

Symptoms are ... You can't log in but you can reset your password with the "Forgot Password", but after you do this you still can't log in.

Mobile authenticators have an option to reset that the user can do, but the keychain authenticators can only be reset by a Blizzard employee.

For convenience here are the phone numbers. I sent email but the response was not received in a timely manner.

Good info, just out of curiosity though how long did it take for the problem to show up?

She didn't specify how long it took for it to happen. In my case I had not logged in in at least eight or nine months. She said if you are not going to play, you should at least log in once a week to make sure it doesn't happen.
